Welcome To EW Capital Mgmt Rental Application

I/We understand the application fee is $50.00 for each person 18 years or older appearing on the application is required. If EW Capital Management, LLC does not perform a credit check, I understand that my application fee will be refunded. So I/WE are aware that all application fee are NON REFUNDABLE once my credit check is completed.

 

I/We understand that Application Fee are Non Refundable.

 

I/We are aware once notification of an approved application, I/We will submit a rental reservation in the form of a money order or cashier check required to hold the property. If I/We choose to cancel for any reason, the rental reservation will be FORFEITED after 72 hours. I/We also understand that rent will commence 15  calendar days from the day my rental reservation has been paid and I must take possession of the property on the date as stated in the fully executed lease agreement.

 

I have read and understand the following information and documents are Required when completing an application to be considered as a complete file:

 

1. Each applicant over the age of eighteen (18) is required to complete a rental application at a cost of $50 per applicant

 

2. Copy of driver’s license or state id

 

3. Employment verification must provided 24 months of employment history for verification. I/We are also aware that I must supply the following documentation:

W2 employee: two current pay check stubs from current employee AND previous year W-2

Self employed: first two pages of last two years 1040 AND two months of bank statements showing deposits,

 

Please note: Some employers use a third party verifier to complete employment verification, this is considered an additional fee paid by the applicant to complete the verification. 

 

4. Proof of income is required if receiving SSI, college stipend for boarding, pension, disability, and others.

 

5. Rental verification: must provide 24 months of residential history including current and/or former landlord  with the full address, landlord full name, and landlord contact information.

 

6. Requirements: Applicant(s) must make 2.5 times the rental rate.

 

7. No eviction/ eviction filing or judgement within the past 24 months.

 

8. IF pets are allowed: (a) Non Refundable pet fee of $250 per pet which is paid at lease signing, (b) No more than 2 pets allowed, (c) No agressive breed(s,) and (d) No pets over 25lbs.

 

9. If the home accepts Housing Choice Voucher, the participant voucher size must equal the number of bedroom the home current.
 

 

I/We understand that we must provide the account numbers for all utilities at the time of move in. No move in will take place without this information. If utilities are now activated on the day of my lease start date, possession will not be surrender and there will be no change in my lease start date.

 

Disclosure Regarding Background Investigation

EW Capital Management LLC (the Company) may obtain information about you from a consumer reporting agency for tenant screening purposes. Thus, you may be the subject of a consumer report and/or an investigative consumer report which may include information about your character, general reputation, personal characteristics, and/or mode of living, and which can involve personal interviews with sources such as your neighbors, friends, or associates. These reports may contain information regarding your criminal history, social security trace, employment and education references, credit history, professional licenses and credentials. You have the right, upon written request made within a reasonable time after receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report. Please be advised that the nature and scope of the most common form of investigative consumer report obtained with regard to applicants for residency is an investigation into your education and/or employment history conducted by Background Info USA, 9990 Coconut Road Suite 314, Bonita Springs, FL 34135, Phone: (800) 403-4398, Fax: (800) 383-7187, or another outside organization. This Disclosure and Authorization allows the Company to obtain from any outside organization all manner of consumer reports and investigative consumer reports now and, if approved for residency, throughout the course of your tenancy to the extent permitted by law. As a result, you should carefully consider whether to exercise your right to request disclosure of the nature and scope of any investigative consumer report.

 

New York and Maine applicants or residents only: You have the right to inspect and receive a copy of any investigative consumer report requested by EW Capital Management LLC by contacting the consumer reporting agency identified above directly.

Acknowledgment and Authorization

I acknowledge receipt of the DISCLOSURE REGARDING BACKGROUND INVESTIGATION and A SUMMARY OF YOUR RIGHTS UNDER THE FAIR CREDIT REPORTING ACT and certify that I have read and understand both of those documents. I hereby authorize the obtaining of consumer reports and/or investigative consumer reports at any time after receipt of this authorization and, if I am approved for residency, throughout my tenancy. To this end, I hereby authorize, without reservation, any law enforcement agency, administrator, state or federal agency, institution, school or university (public or private), information service bureau, employer, or insurance company to furnish any and all background information requested by Background Info USA, 9990 Coconut Road Suite 314, Bonita Springs, FL 34135, Phone: (800) 403-4398, Fax: (800) 383-7187, another outside organization acting on behalf of the Company, and/or the Company itself. I agree that a facsimile (fax), electronic or photographic copy of this Authorization shall be as valid as the original.

 

California applicants only: Please check this box if you would like to receive a copy of an investigative consumer report or consumer credit report if one is obtained by the Company at no charge whenever you have a right to receive such a copy under California Law.
